In order for us to prepare all the necessary forms to be filed to the government authorities we will need the below questionnaire pre-filled by the employer at the destination country.

Have insolvency proceedings been opened against the employers assets or the assets of the receiving branch office in the last five years?
Have insolvency proceedings been opened against the employer's assets or the assets of the receiving branch office for lack of assets in the last five years and have business operations been discontinued?
Is there a family relationship between the owner or manager of the enterprise and the prospective foreign worker
